Record what the interaction was about.
Note the advice or guidance provided during the conversation.
Key Points:
Behaviour: Describe the service user’s demeanour or behaviour during the interaction.
Risk Level: Assess and note the risk level (e.g., low, moderate, high) based on observations or disclosures.
Progress: Document any progress the service user has made toward their goals.
Referrals: Note any referrals made to other services, programs, or professionals.
Actions Taken:
Include actions you’ve taken, such as submitting a SERVACT, escalating concerns, sending letters, or other interventions.
